IMPORTANT:

Click and save the Forms as an image to your desktop for printing. PLEASE WRITE ELIGIBLY, ESPECIALLY YOUR NAME AND EMAIL ADDRESS!

Each participant is required to have an individual release form. When uploading the release form, name the FILE ‘STUDENT’S NAME’ RELEASE. For Example: ‘Jane Doe Release’

For the Entry Form, save the form with your ‘TEAM NAME.’ For Example: “Be Street Smart!”  Only one Entry Form is needed for the group.

Entries submitted without Entry and Release form are not eligible.
